The Department of Health has launched a National Patient Safety Incidents Survey.
The survey is now open to patients, family members, advocacy groups, healthcare staff, service providers and the public.
Hearing your views and experiences will help us to examine, learn and improve how we can manage complaints and patient safety incidents in healthcare. There are two ways you can take part in this research survey.

The survey is being carried out by Crowe, the research company that is working on this project. The survey is anonymous and will be open until 17th June 2022 at 5pm .
Some people may be interested in taking part in a one-to-one phone interview to give more detail about their views and experiences of patient safety incidents.

After you register, you may be contacted by a researcher from Crowe, the company that is working on this project. The phone conversation will take up to 20 minutes.
Please note this is only an expression of interest and you may not be called to tell your story, but we very much appreciate your interest in the project.
The survey and interview may be used to inform the development of a national Patients Safety Incidents Policy.
The purpose of the carrying out research is to gather information on the values and culture that exist in healthcare in managing patient safety incidents.
Further research on management of complaints in healthcare is planned to take place at a later stage in 2022.
Patients, families, advocacy providers and staff who use and work in the health service have important views from experiences of patient safety incidents. Some people may have had good experiences, while other people may feel that things could have been done better. We want you to tell us what works well and what can be learned to make improvements. Information from the research study will help to inform and plan national policy and add to the body of knowledge regarding complaints and patient safety incidents in health care services.
