1. Mr. [ ] (‘the Applicant’) has made a claim for compensation under the Scheme of Compensation for Personal Injuries Criminally Inflicted (‘the Scheme’).

2. The application relates to an incident which occurred on the [ ] at [ ]. The Applicant states that he suffered dental injuries comprising tooth loss and a fractured pallet as a result of an assault. The Applicant reported the incident to Gardai at [ ] Garda station.

3. The Tribunal acknowledged receipt of the Applicant’s application form on the [ ] and requested a copy of the Applicant’s P60, a statement from the Department of Social Protection and original receipts in relation to expenditure incurred as a result of this incident. No receipts/vouchers, details of loss or earnings, nor medical reports have been furnished. No Garda report appears on the Applicant’s file.

4. Correspondence issued to the applicant’s Solicitors by the Tribunal on the [ ] requesting confirmation as to whether the Applicant was pursuing this application. No response appears to have been received thereto.

In the absence of the provision of sufficient information by the Applicant in this case it is not possible to make a determination in relation to the matter. Therefore, no award is made pursuant to paragraph 10 (previously 11 of the Scheme).
