If you are a customer of HR Shared Services who cannot access the Self-Service portal for the following reasons, please use the Excel application form below:
- Remote worker
- Long-term sick leave
- Extended leave of absence

However, please note that this will need to be printed, filled in by hand, and either:

We would therefore advise that where possible, you complete the Excel form rather than the PDF.
Staff members who are not HR Shared Services customers but who are on HRMS
If you wish to apply for mobility and are an employee of the following, please use the Excel application form below:
- Garda Civilian, An Garda Síochána
- Irish Prision Services
- House of the Oireachtas
- the Ombudsman for Children or
- the National Council for Curriculum and Assessment
If you do not have access to Microsoft Office in order to complete the Excel form, you can download a PDF application form.
However, please note that this will need to be printed, filled in by hand, and either scanned or posted to your Local HR Division.
We would therefore advise that where possible, you complete the Excel form rather than the PDF.
