Application form for re-approval of an EQUINE breeding programme under Regulation 2016/1012

Please attach a copy of

• details and supporting documentation for any changes to the information provided in your application for recognition as a breed society or breeding operation including any amendments to your (a) legal personality and/or (b) memorandum and articles of association, etc.
• the breeding programme referred to in this application. This breed programme should cover at a minimum points referred to in Annex I, Part 2 and Part 3 (equines only) of Regulation (EU) 2016/1012
• number of breeders and breeding animals covered by breeding programme within Ireland and other geographical territories (as relevant) in the most recent 3 years in the format provided (complete Appendix 1 attached). Further information on numbers may be requested by DAFM from time to time
• copy of a signed agreement(s) with third party where specific technical activities are outsourced (where relevant)
• a copy of the zootechnical certificate that meets the content and format as outlined in the appropriate Commission Implementing Regulation 2017/717 and Commission Delegated Regulation 2017/1940 as relevant.

Please provide a separate application form for each breeding programme seeking re-approval.
