Fuel Allowance: Over 70 Application Form (NFS2 O/66)

This form is used to apply for the Fuel Allowance under the National Fuel Scheme. This form is for Fuel Allowance applicants who are aged 66 years or over only. Please do not use this form if you are under 66. If you are under 66, you can apply for the Fuel Allowance using the NFS1 Application Form
