Request an application form


If you need to apply for a social welfare payment, you can do this online or by downloading or requesting an application form.


Apply on MyWelfare

MyWelfare is the online home of social welfare services. It is safe, secure, and accessible anytime, anywhere, and on all devices.

All you need is an email address, PPSN and bank account details to use these services. You will have to set up a basic MyGovID account, but it is a simple and straightforward process.

If you would prefer to apply for a social welfare payment in writing, many of our application forms are available to download on gov.ie.

You can search for the application form you need by using our list of Social Welfare Forms.

You can also search for an application form by using our list of Social Welfare Schemes and Services.

  • select the relevant scheme or service
  • if the application form is available to download, it will be in the Apply section of the page

If you print from the website, you will not need to wait for an application form to arrive in the post.
