Pre-employment medical forms

The Public Appointments Service/employing department will advise you of the pre-employment medical requirements for the post.

Any additional documents specific to the post (for example, an audiogram or vision test) should be forwarded to: The Office of the Chief Medical Officer, Floor 5, 90 North King Street, Smithfield, Dublin 7.
