The Sláintecare Healthy Communities (SHC) Programme is a Healthy Ireland initiative to reduce health inequalities in Ireland. Launched in 2021, the Programme provides health and wellbeing services and community development initiatives in 19 community areas across Ireland where health inequalities are most evident.
The Sláintecare Integration Fund End of Programme Report 2022 sets out the notable achievements reported by the 123 projects funded through the Sláintecare Integration Fund. The projects tested new ways of working and delivered innovative models of care, bringing care closer to home by putting the patient at the centre of service design and delivery. The significant impacts of the projects on patients and the healthcare system are highlighted throughout the report. It sets out the hospital admissions avoided, Emergency Department attendances avoided or the reduction in waiting lists. It also highlights the impact felt by patients and their families where waiting times improved or journey times to receive treatment reduced.
This 2022 end of year report sets out the progress in improving access, affordability and quality in our health and social care services and in the measures taken that are making an immediate impact on the lives of patients, as well as working to deliver foundational longer-term change that will significantly enhance overall capacity and patient centred care.
The Sláintecare Action Plan 2022 sets out the ongoing reform priorities aligned with the Sláintecare Implementation Strategy & Action Plan 2021 – 2023. The plan focusses on progressing the implementation of the two key reform programmes, Improving Safe, Timely Access to Care and Promoting Health & Wellbeing, and Addressing Health Inequalities – Towards Universal Healthcare.
