Important Notice

The Department is currently facing an unprecedented challenge in the face of the public health emergency arising from COVID-19. To deal with the huge demand on our services it has been necessary to re-assign staff across the organisation to key front line areas.

In light of this, it is regrettable that the Department is not currently in a position to respond to Subject Access Requests within the normal deadlines. In the light of this unprecedented crisis, we would be grateful if you could consider withholding your request until further notice. We will continue to monitor this situation as it evolves and provide further updates as appropriate.

For Subject Access Requests accepted by the Department prior to the COVID-19 emergency, the Department will advise requestors in cases where it will unable to meet the GDPR deadlines.

To make a Subject Access Request (SAR), please download the Subject Access Request Form and email the completed form to SARS@welfare.ie or post to:

Proof of identity must accompany this Subject Access Request form.


Requests for personal data must be accompanied by proof of you identify, including a photocopy of your passport or driving licence or Public Services Card, so that we can verify your identity.

Requests for personal data should include:

  • your name, PPS number, and postal address
  • your signature and the date of the request
  • if you are applying on behalf of another person, then a signed authority from the individual is required
  • any details which may help us locate the information which is the subject of your request
